1. Eligibility through the Previously Untreatable category is limited to coronary sinus lead failures or left ventricular lead failures/difficulties. 2. Eligibility through the High Risk Upgrade category is limited to patients who are ineligible for a standard CRT upgrade due to known relative contraindications. • Refer physician to Medical Affairs (med.affairs@ebrwise.com) if there are further questions. • Reference the common IFU for approved indications.
